1. click in the Market on download and install the Theme
2. click on the Menu Button on your Phone at the Homescreen and choose Themes
3. search for the MIUI X4 Theme and click on "Apply"
NOTIZ! Usually an update should be displayed automatically. If you don't see the new Icons just kill GoLauncher or try to switch the theme and back to MIUI X4 Theme.

But, I've some missing icons including Can Knockdown 3, Currency converter 2.0 (the one with white-green calculator icon) and Calculator++. Can u add 'em plz? A lot of theme makers will just abandon their themes after initial release and refuse to update them. I remember paying for a theme only to have it not work on my new S3. The developer refused to update it in order to fix the issue and practically stole my money. It is extremely pleasant on the eyes with great colors and icons. A huge plus is that it works on so many launchers, as I've experimented with a couple of them and this works on both. The theme seems to fit best on GO Launcher in my opinion, but looks great on ADW as well. I am yet to find a theme I like more. Even the free version has more than enough icons to make me happy. My only complaint is that the theme slows my phone down moreso than other themes.
